Private Duty Nursing

Hays CISD provides school nursing and school health services for students on all campuses. Each campus has a registered nurse to provide health services. Due to the complex medical needs of some students, parents may wish to waive district school nurse and school health services during a 504 or ARD.  In the event a parent wishes to utilize their own private duty nurse, the parent(s) are financially responsible for this nursing care. In order for private duty nursing services to be initiated by a parent, the parent must complete an agreement acknowledging the waiver of school health and nursing services by district staff PRIOR to a private duty nurse beginning care on campus.

It is the district's goal to work together to provide the best and safest educational experience for all students. In order to provide clear guidelines and expectations to improve the safety of the children on campus and eliminate miscommunication, please refer to the Hays CISD Private Duty Nursing Guidelines (Spanish) for further information regarding private duty nursing care.

How to request Private Duty Nursing Services

In order to request private duty nursing services for a student on campus, the 504 or IEP must indicate the parent waives district nursing services and will provide their own private duty nurse.

A request for private duty nursing services can be initiated by following the steps below; please note, there are requirements for the parent/guardian AND the private duty nurse/agency. All steps MUST be complete PRIOR to a private duty nurse beginning services on campus. After initiation of services, all steps must be completed prior to the start of each school year, when there is a change in the nursing provider, or prior to a new student being served. Parent/Guardian Steps for requesting private duty nursing services:

  1. Complete the Parent/Guardian request for Private Duty Nurse on Campus form.

  2. Read the Hays CISD Private Duty Nursing Guidelines (Spanish)

  3. Watch the Hays CISD Private Duty Nursing Orientation.

  4. Complete the Hays CISD Parent Agreement for Private Duty Nursing Services via ReadySign. An executed copy will be emailed to you once signed by a district representative.

  5. The 504 or IEP for my student indicates I am providing a private duty nurse and waiving district health services while the private duty nurse is on campus. I understand that I am responsible for this expense.

Private Duty Nurse/Agency Steps for requesting to be on campus:

  1. Complete the Private Duty Nurse Request for Background Check form. Hays CISD Human Resources will provide a FAST Pass for the nurse to schedule fingerprinting after this form is submitted.

  2. Read the Hays CISD Private Duty Nursing Guidelines (Spanish)

  3. Watch the Hays CISD Private Duty Nursing Orientation.

  4. Complete the Hays CISD Private Duty Nursing Services Nurse Agreement via ReadySign. An executed copy will be emailed to you once signed by a district representative.

  5. The Private Duty Nurse MUST have a completed AND cleared background check through Hays CISD Human Resources PRIOR to initiating services on campus.
